Care Data – sharing information (going live shortly) It is important that the NHS can use information from medical records to get a complete picture of what is happening across health and social care and to plan services according to what works best. The new system delivered by the Health and Social Care Information Centre (HSCIC) and NHS England on behalf of the NHS will provide joinedu0002up information about the care received from all of the different parts of the health service, including hospitals and GP practices. Your date of birth, full postcode, NHS Number and gender rather than your name will be used to link your records in a secure system, managed by the HSCIC. Once this information has been linked a new record will be created. This new record will not contain information that identifies you. The type of information shared, and how it is shared, is controlled by law and strict confidentiality rules. The new system will also provide information that will enable the public to hold the NHS to account and ensure that any unacceptable standards of care are identified as quickly as possible. All staff will ask your permission before they look at your Summary Care Record. Having a Summary Care Record will mean that if you have an accident or become ill anywhere in England, the doctors treating you will have immediate access to important information about your health. Oxfordshire Care Summary: (Our patient records should be available to this service by the end of 2013) The Oxfordshire Care Summary is a single electronic view of specific, up-to-date, clinical information from your GP record and other records which will be kept to support your care within NHS organisations in Oxfordshire. Authorised clinical staff will be able to see any significant diagnoses you may have, what medication you take, or have taken recently and what tests your GP has carried out. They will not be able to see information about conversations you have had with your GP or information on sensitive subjects such as sexual health. The record will also include information about treatment you have received at the Horton or Oxford University Hospitals, attendances at Minor Injury Units and GP Out of Hours services, and some documents such as care plans. This information will be used to ensure you get the safest treatment as quickly as possible within Oxfordshire.
