TRAVEL RISK ASSESSMENT

Please take the time to read the Travel Appointment Information which gives details of our Travel Risk Assessment.

You will then need to fill out the on-line form and return this to the Surgery. You will then need to book a telephone appointment with one of the Practice nurses for a week after you have returned the form.
