New Patients

To register as a patient at Church Street Practice, please download and complete both the Registration form and the New Patient Questionnaire (see below). These forms are also available from the surgery.

You will need to bring both forms into the surgery with some form of photo-ID e.g. passport or a new style driving licence. If you have only recently come to the UK, you will need to provide evidence that you are staying for more than 6 months and are working, looking for work or in education.

All new patients are asked to make an appointment with the Practice Nurse for a routine health check.

We operate an open list system which means you may choose which doctor you see on any occasion; for instance, for a particular problem you may prefer to see a female GP or the GP who has a special interest in that area, rather than your usual doctor. We strongly recommend that, if you have an ongoing problem, you see the same doctor as continuity is an essential element of good quality care.
