| Prescriptions |
|
The
practice has a Prescription Service for Repeat Prescriptions
and queries.
It is open 09:00 to 17:00 on
weekdays. This service does not dispense
medication.
An
out of hours chemist service is available at Lloyds Chemist
in the Health Centre until 19:00 on weekdays and 09:00 - 13:00
on Saturdays. There
is a rota system for Sundays and Public Holidays. Please

Repeat Prescriptions
There
are several ways available to you for making repeat prescriptions
requests:
- Tick
the items required on your prescription counterfoil and
mark the chemist you wish to collect your medication from
and hand it in or post it to the surgery. If you have lost
the counterfoil, jot the details of your medication on a
piece of paper but do remember to include your name,
address and date of birth.
- Leave
your prescription request with your local pharmacy, and
they will bring it in to us.

We
try to ensure prescriptions are ready for collection within 48
hours but please remember that a little more time is needed if
you are leaving you prescription request with one of the local
pharmacies for delivery to the Health Centre. We can send your
prescription direct to any of the local pharmacies, if this is
more convenient for you, so please do remember to tell us
where you want to pick it up.
If
one of our prescriptions teams is not available to take your telephone
call you will be invited to leave them a message. The messages
are checked regularly during the day so your request will
receive prompt attention. Do please remember to leave your
name, address and telephone number as well as full details of
your medication or query.
Please
remember that messages
left on the answer phone after 17:00 on Fridays will not be
addressed until Monday morning.

Contraception Services |
| All
the doctors and the Specialist Practitioner are fully trained
in family planning and contraceptive advice.
This includes natural methods, barrier methods, the
pill, intra uterine coils, injectable and emergency
contraception. |
Emergency Contraception |
| The
‘morning after’ pill can be effective when taken up to 72
hours after unprotected intercourse.
We are happy to see you in the surgery or consult over
the telephone to prescribe if
necessary. It is also possible to be seen over the weekend or
on bank holidays by calling the out of hours service by
calling 0845 345 8995. An alternative
method is fitting a coil (which can be done up to 5 days after
unprotected intercourse). |
Home Visits |
| A
home visit may be appropriate if you are too ill to come to
surgery. If you
need a home visit please try to telephone the Surgery on 01235 770245
before 10:30 am and your details will be taken by the
Receptionist. Home
visits are very time consuming and we would, therefore, ask
you to consider carefully whether or not a visit is necessary. |
Out Of Hours Cover |
|
The
Surgery is open from 08:30 – 18:30 Monday to Friday.
If you need a doctor in an emergency after these hours,
or on a Saturdays, Sunday or Bank Holiday, call 0845 345 8995 for the out
of hours service.
The
out of hours emergency cover is run by Oxfordshire Primary
Care Trust.

NHS Direct Advice Helpline |
|
Patients
may choose to contact NHS Direct for health information or
advice.
NHS
Direct is a 24-hour nurse led telephone helpline and has been
designed to complement traditional primary care services.
|

The
service aims to help callers by providing them with the
advice, information and reassurance they require to look after
themselves at home if appropriate.
It will also ensure that those callers who need further
professional help are directed to the right service at the
right time. The
service is provided by experienced nurses with the appropriate
training and under clinical supervision. NHS Direct can also
give you details of NHS Dentists in the area. |
Advice And Test Results |
|
The
Doctors are glad to give advice over the telephone and have a
telephone appointment times built into their surgery. Please
let the receptionist know if your problem can be dealt with
over the telephone.
Please
call for results of investigations between 14:00 and 16:00 when staff will have more time to help you.

| Health
Promotion |
| This
Practice is involved in and committed to health promotion
through routine screening and recall systems.
From time to time we are involved in research projects
and you may be invited to participate.
Your care will not be compromised should you prefer to
decline. |
Travel Advice/Vaccinations
|
| If
you are planning to travel abroad, please make an appointment at
least 8 weeks in advance to enable the Practice Nurse to arrange
a programme to ensure adequate protection.
|
| Training
In Primary Care |
|
Church
Street is a training practice for medical students from the
Oxford Medical School. When
a student is with the doctor, a notice indicating this will be
displayed at reception. Please
let the receptionist know if you would rather see the doctor
alone.
Church
Street is also a training practice for the training of qualified doctors to become general practitioners.
The Doctors are Registrars, who have spent four to five years
in hospital appointments before joining the practice and will
work as a member of the team.
You may be offered an appointment with our registrar.
There may also be Nurses, Health Visitors, Midwives and
other staff in training with the practice

Suggestions And Complaints
|
|
Constructive
suggestions to improve our service are always welcome.
Should you have any queries or complaints regarding the
Doctors, staff or treatment you have received, please do not
hesitate to write or email
to Mrs Sheila Dearman the Practice Manager
or Dr Paul Bryan the Senior Partner.
We
have a practice based complaints procedure and a leaflet
explaining this procedure is available at Reception.
This process should be two way between patients and the
Practice. Just as
patients have a right to be treated with respect by staff, the
staff have a right to be treated courteously by patients.
Care is best delivered in an atmosphere of mutual
respect.

The Church Street Care Fund |
| A
charity fund has been set up for the benefit of patients
registered with the practice, to provide equipment and
facilities that may otherwise be unobtainable. Please enquire
at Reception for details. |
Patient Involvement Group (P.I.G.) |
| Our
national award winning P.I.G. work hard to improve the
exchange of information, ideas and concerns between the
patients, the doctors and their team.
It is run entirely by patients for patients and any
input would be welcome. For

Private Fees |
| Non-National
Health work will attract a fee in line with that suggested by
the British Medical Association.

Disabled Access |
| There
is wheelchair access at all entrances to the building and all
the consulting rooms are on the ground floor.
A disabled patient toilet is situated adjacent to the
waiting room. Disabled
parking bays are adjacent to the side entrances. There is also
a hearing aid loop available in the practice. |
Patient Library (In the Quiet Room) |
| We
have a small library of books and videotapes, which can be
borrowed by our patients.
There are also several leaflets on common disorders,
procedures and health issues generally available.
